Intending Yatris will be reporting to reception in between 9:30 AM to 3:00 PM and then they will get registered with all the details including collection of their Aadhaar Cards copies.
Registration not to be done for Yatris who is below the age of 13 Years or above the age of 70 Years and to pregnant lady with 6 weeks pregnancy.
CONSULTATION DAYS & TIMINGS:
All days except Sunday.
Consultation with Physician, with all reports of investigations next day between 11:00 AM to 4 PM.
PROCESS OF CERTIFICATION
Consultant will take detailed medical history, do detailed examination of Yatri and review all his reports of investigation done.
If any other investigation needed then Yatri will be told and will be charged separately.
If any referral Consultants visit needed then Yatri has to pay separately.
Consultants will finally certify on the prescribed format (if found to be fit).
The Certificate will be duly signed and stamped with date and time.
The Certificate will be further duly signed and stamped by Nodal Officer Dr. Sneh Jain.
